Phelps Case Management Consultants provide quality, clinically sound case management. Using a holistic approach to improving individuals quality of life while monitoring outcome measures during rehabilitation following life changing injuries.
Specialising in working with those impacted by brain injury, complex psychological injuries and/or mental health challenges; we also provide Vocational Rehabilitation services - we have been awarded Team of Year 2023 by the Vocational Rehabilitation Association. We work to support people to meet their goals and guiding individuals through their recovery by working to goals that are meaningful to the person as well as important to maximising their recovery and independence.
Phelps Case Management Consultants also offer support and guidance via bespoke trainings and personalised supervision to professionals in the case management field. Registrations held in the team include Case Management Society UK, British Association of Brain Injury & Complex Case Management, Vocational Rehabilitation Association UK as well as being an Institute of Case Management Supporter.

Specialist Case Management
With extensive experience in working in the field of brain injury recovery and psychological injuries; services start with an Initial Needs Assessment leading to specialised, comprehensive input tailored to the needs of the client throughout the pathway to recovery. Multi-disciplinary teams are built to ensure all possible progress is made to address impactful ongoing symptoms and to meet all goals of the injured party.

Multi-trauma Case Management
Multi-trauma case management services include Initial Needs Assessments and ongoing case management input to maximise recovery with guided rehabilitation. Working toward goals developed with the client, input is facilitated to improve function and reduce the impact of symptoms experienced. A vocational focus can be imbedded in input provided where appropriate.
Vocational Services
Working under an ‘employment first philosophy’; Vocational Rehabilitation Services can be provided through standard programmes or through bespoke input alongside other case managers. Every person needs a purpose in their day and with vocational input an individual can work towards being on a level playing field during or after their clinical/medical rehabilitation.

Training & Support
Coaching and support packages can be formulated to address ongoing professional development and case management skills to other professionals in the field. Trainings can be provided to direct care staff and teams. Coaching, supervision and reviews can also be arranged. Please enquire for any training or support needs.
